a) Payment can be via cheque, atm or i-banking.

b) Please have ready the cheque number or ib/atm transaction reference number and fill in details below before submitting this form.

c) Cheque payment: Please make the cheque out to Causes for Animals (Singapore) Limited and mail to c/o 80 Kings Road, Singapore 266462. Write your name, email and (if applicable - name of com care animal) on the back of the cheque.

e) Receipts will be emailed.

f) Donations are not tax-deductible.

G) Any donations left after bills are paid will be transferred to help other animals is need
